I would like to become a member of Forever College. I have filled out this application completely and accurately. I am submitting dues along with my application and agree that this information will be held on my record for as long as I am a member. Membership fee is non-refundable and will be used for the following: $150 Year - discount card + merch box + 25% FC merch + $50 merch credit, $75 Per Semester - discount card + merch box + 25% off FC merch, $20 - access student discount card + FC sticker + 10% FC merch.
